Understand the membership, insurance billing, provider access, and enrollment process in one place.
What is SEVA concierge membership?
A voluntary membership for enhanced access, longer appointments, prevention planning, and the additional non-covered services listed in your agreement. It is not insurance and is not a direct primary care agreement.
What is the membership price?
Primary membership is $2,999 annually or $299 per month ($3,588 over 12 months). Adult dependents over 18 are $2,499 annually or $225 per month ($2,700 over 12 months). Dependents ages 2–18 are $1,999 annually or $189 per month ($2,268 over 12 months). Dependent eligibility is confirmed during enrollment.
Does membership replace insurance?
No. Active health insurance is required. Concierge members have no co-pay or deductible for office visits. Insurance cost-sharing may apply to other covered services.
Will office visits still be billed to insurance?
Medically necessary covered office visits may be billed to insurance. Concierge members have no co-pay or deductible for office visits. Non-covered membership services are not also billed to insurance; costs for other services are reviewed separately.
How do members contact a provider?
Use the Spruce Health app for secure text, phone, and video communication. Weekday messages route to your provider between 7 a.m. and 3 p.m.; secure text is the preferred method. General office hours are Monday–Friday, 8 a.m.–4:45 p.m.
Who responds after hours?
A qualified on-call provider covers after 3 p.m., overnight, weekends, and holidays. This may be someone other than your primary provider. Responses may be delayed, and messaging must not be used for emergencies. Call 911 for a medical emergency.
Are same-day appointments guaranteed?
No. Same-day or next-day weekday access is the goal, subject to availability and clinical appropriateness. Contact us early—ideally before 3 p.m.—for sick visits or urgent requests. Your team may direct you to urgent care or an emergency department when appropriate.
Why is SEVA offering concierge care?
Patients asked for more time, easier communication, and a proactive approach to wellness. A smaller concierge panel supports closer relationships, fuller evaluations, and more individualized follow-through.
Can I have as many visits as I need?
You may request medically appropriate in-person or virtual visits as often as needed. Frequency, duration, and scheduling depend on clinical need, provider discretion, and availability. Concierge members have no co-pay or deductible for office visits. Costs for other services are reviewed separately.
Will I always see my provider?
We make every effort to have your primary concierge provider evaluate and treat you. When your provider is unavailable, another qualified provider may see you so you can continue receiving care. We’ll explain how coverage works when you enroll.
Are the wellness evaluations twice a year?
Yes. Membership includes comprehensive concierge wellness evaluations twice per year, along with concierge lab enhancements and EKG as medically appropriate. Testing is selected by your clinician; additional covered testing may be billed to insurance.
Are all labs and medications included?
No. The agreement defines specific concierge lab panels and selected in-clinic services. Other labs and most ongoing medications are separate. Selected acute-care medications may be included at provider discretion, modified, or separately billed where required by law or payer policy.
Can children and family members join?
Yes. The agreement includes rates for adult dependents and dependents ages 2–18. Each person needs an enrolled membership; clinical appropriateness and dependent eligibility are confirmed by the practice.
Can you help while I’m traveling?
Telemedicine depends on where you are physically located and what is legally permitted at the time of care. We can help coordinate next steps locally. The FAQs also describe ordering labs at nearby U.S. facilities when appropriate.
Are hormone and weight-management treatments included?
There is no blanket inclusion. These services may involve separate medication, procedure, visit, and monitoring costs. Selected member discounts are available; we’ll review the specific service and costs before care. Prescriptions require appropriate clinical evaluation and follow-up.
Can I use HSA or FSA funds?
Eligibility depends on the service and your plan. Confirm directly with your plan administrator before using these funds; membership should not be assumed eligible.
How do cancellation and renewal work?
Membership renews monthly or annually according to your billing option unless cancelled. The agreement says monthly fees are non-refundable and provides a prorated refund for unused annual membership after 30 days’ written cancellation notice. Your enrollment agreement governs the details.
How do I join?
Arrange an introduction, review the membership agreement and billing arrangements, and complete enrollment with the team’s guidance. Benefits begin once the agreement is effective and the first payment processes successfully.
Can you help with referrals and hospital coordination?
Yes. Membership includes enhanced coordination and communication with specialists and hospital teams when needed. Specialist and hospital services are separately billed; membership does not replace hospital or emergency care.
How does SEVA’s approach compare with functional medicine?
We share an emphasis on the whole person: understanding your history, nutrition, sleep, activity, stress, and potential contributors to symptoms. We combine that perspective with evidence-based family medicine, appropriate testing, medications, and referrals. Looking for underlying causes and supporting lifestyle changes are also part of comprehensive primary care.
SEVA is a concierge family medicine practice. Our clinicians have not completed formal functional or integrative medicine training, and we do not claim functional medicine certification. We focus on individualized care within our training and scope, without promising to identify a single root cause for every condition.
