This website describes SEVA Concierge Medicine and provides general health education. It does not diagnose a condition, provide individualized medical advice, or establish a provider–patient relationship.
Membership and treatment
A membership inquiry is not a medical appointment. Your signed membership agreement governs enrollment, fees, included benefits, renewal, and cancellation. Membership is not health insurance. Concierge members have no co-pay or deductible for office visits. Covered services may be billed to insurance; cost-sharing may apply to other services.
Treatment decisions require a clinical evaluation. Availability, suitability, and costs are reviewed individually. Educational descriptions of medications or research do not establish that a treatment is available from SEVA.
